What kind of base plate is this? And where can I buy one?

I have this base plate I bought used a few years ago and I need to buy a new one. It is used with 15mm rails. Anyways here is a pic. I need one the exact same. But for the life of me I can't find who makes them. Any help would be Photo on 2010-10-27 at 22.03.jpg!

That looks like either the old-style system from Cinevate or maybe Cine City (can't remember which).

Wonder where I can get one.. hmmm... Because I need it to be able to swivel away from my rails.. Like shot away from the rails like a T instead of shooting down the rails like normal..
